REGISTRATION INFORMATION Youth Registration form: Word .pdf Adult Registration form: Word .pdf Early Bird Registration Fee: $250 paid in full with completed registration forms by May 15, 2008 Registration Fee: $275 paid in full with completed registration forms May 16-31, 2008 Fee includes all meals (starting with dinner on the first day and ending with breakfast on the last day), lodging, workshops, camp materials, all activities and an earned camp T-shirt. − Registration from 10 am − 12 pm on Tuesday. − Lunch will NOT be provided the first day. − Camp festivities will begin promptly at 12:30 p.m. Payment options: Checks, money orders, purchase orders, Visa and/or MasterCard will be accepted with registration forms as form of payment. Make payable to Extreme Youth Leadership Training Camp. Cancellations: No refunds will be given, but substitutions will be accepted no later than two weeks prior to the first day of camp. If a participant cancels within that time and would like another student to go in their place, only a male student can replace another male student and a female only can replace a female student to avoid any problems with lodging. Any substituted participant must have a completed registration form and medical release/insurance form. HINT: Encourage fundraising efforts so the kids are responsible for some of their own money. The training camp experience belongs more to them if they have had a stake in raising some of their own registration fee. Size Limitations: Due to limited campus housing, the number of attendees is restricted. Applications will be accepted on a first-come, first-served basis. |
